The tradition of changing place (and person) names to a more "English" form dates back to the Middle Ages at least. This is called Anglicisation.
The alternate namings arent't just limited to English speakers. During the opening ceremonies of the 2006 Olympics, many of the country name on the standards that were carried before the athletes as they entered the stadium had Italian versions of their true names. For example, the United States was shown as Stati Uniti d'America and the Netherlands was shown as Paesi Bassi.
